How it works:

1. Fill out the above information.
2. Ship your regulator, control board, or keypad to us via your preferred carrier.
3. We will contact you when repairs are completed to arrange payment and return shipping.

Notes:

• Rush Service: When sending repair items overnight for same day RUSH service DO NOT use Express Mail from the Postal Service. It does not arrive until mid afternoon at our location. Only use UPS Next Day Air or Federal Express Priority for rush repairs.
  Please CLEARLY write "RUSH" on this paperwork if you require rush service. A 25% fee will be applied to each rushed item.


• Abandoned Items: Items sent to Flight Systems Inc. for test and/or repair will be held for a period of six months from the date received. The items will be discarded if there has been no response from our attempts to contact the sender.
